Does a community have more of a responsibility to local public health or global health?

It seems we have huge efforts helping people abroad (globally) when there are local people who need our resources just as much. What are your opinions? Why should local community health be more important than global health? All opinions are welcome.

    The community heath should be more important than funding global health this is less expensive in the community heath should be more important.
    The long run for the long run for the community heath should be more important than people who live abroad.
